•HCP can apply online for the patient or download application
•Receipt of incomplete applications can lead to delays in processing
•Completed forms and required documentation can be uploaded to the OPAF Care Connect Portal, mailed to: Otsuka Patient Assistance Foundation, Inc., PO Box 501878, San Diego, CA 921501878; or faxed to: 18447276274
•Health care professionals must refer to the ICD10CM codes when completing the form
•Eligible patients will receive medication free of charge shipped to their doctor's office, unless doctor advises of different shipping address, for up to one year
•Patients receiving government issued insurance (Medicare, Medicare Part D, Medicaid) will need to reapply at the end of every calendar year
