Dosing
Asthma (12 years and older) 2 inhalations of 80 mcg/4.5 mcg or 160 mcg/4.5 mcg twice daily by oral inhalation; starting dose based on asthma severity.
Asthma (6 to <12 years) 2 inhalations of 80 mcg/4.5 mcg twice daily by oral inhalation.
Chronic Obstructive Airway Disease, Bronchitis, Chronic, Pulmonary Emphysema 2 inhalations of 160 mcg/4.5 mcg twice daily by oral inhalation.
Asthma (>=12 years) 2 inhalations twice daily of DULERA 100 mcg/5 mcg or 200 mcg/5 mcg via oral inhalation; maximum 2 inhalations of 200 mcg/5 mcg twice daily (800 mcg/20 mcg per day).
Asthma (5 to <12 years) 2 inhalations of DULERA 50 mcg/5 mcg twice daily via oral inhalation; maximum daily dose 200 mcg/20 mcg.
Contraindications
- Primary treatment of status asthmaticus or other acute episodes of asthma or COPD where intensive measures are required
- Hypersensitivity to any ingredient in BREYNA
- Primary treatment of status asthmaticus or acute episodes of asthma requiring intensive measures
- Known hypersensitivity to mometasone furoate, formoterol fumarate, or any ingredient in DULERA
Adverse Reactions
Most common (>=3%) Nasopharyngitis, headache, upper respiratory tract infection, pharyngolaryngeal pain, sinusitis, oral candidiasis, bronchitis, influenza
Serious Serious asthma-related events (hospitalizations, intubations, death), cardiovascular and CNS effects, Candida albicans infection, pneumonia or lower respiratory tract infections in COPD, immunosuppression, hypercorticism, adrenal suppression
Postmarketing Angina pectoris, tachycardia, atrial fibrillation, cataract, glaucoma, oropharyngeal candidiasis, anaphylactic reaction, hyperglycemia, hypokalemia, tremor, dysphonia, hypotension, hypertension

Postmarketing Angina pectoris, cardiac arrhythmias (atrial fibrillation, ventricular extrasystoles, tachyarrhythmia), vision blurred, anaphylactic reaction, angioedema, severe hypotension, rash, pruritus, QT prolongation, blood pressure increased, hypokalemia, hyperglycemia, asthma aggravation (cough, dyspnea, wheezing, bronchospasm).
Pharmacology
Budesonide is an inhaled corticosteroid that suppresses inflammation by inhibiting multiple cell types and mediators involved in allergic and non-allergic inflammation; formoterol is a long-acting selective beta2-adrenergic agonist that relaxes bronchial smooth muscle by stimulating adenyl cyclase and increasing intracellular cyclic AMP.
DULERA combines mometasone furoate, a synthetic corticosteroid with potent anti-inflammatory activity that inhibits multiple cell types and inflammatory mediators involved in the asthmatic response, and formoterol fumarate, a selective long-acting beta2-adrenergic receptor agonist (LABA) that relaxes bronchial smooth muscle by stimulating adenyl cyclase and inhibiting mast cell mediator release.
