Dosing
Chronic idiopathic constipation 2 mg orally once daily; reduce to 1 mg once daily in patients with severe renal impairment (creatinine clearance less than 30 mL/min).
Chronic idiopathic constipation, irritable bowel syndrome with constipation 3 mg taken orally once daily, with or without food.
Contraindications
  • History of hypersensitivity to MOTEGRITY (reactions including dyspnea, rash, pruritus, urticaria, and facial edema have been observed)
  • Intestinal perforation or obstruction due to structural or functional disorder of the gut wall, obstructive ileus, or severe inflammatory conditions of the intestinal tract such as Crohn's disease, ulcerative colitis, and toxic megacolon/megarectum
  • Age less than 6 years (risk of serious dehydration)
  • Known or suspected mechanical gastrointestinal obstruction

Pharmacology
Prucalopride is a selective serotonin type 4 (5-HT4) receptor agonist and gastrointestinal prokinetic agent that stimulates colonic peristalsis via high-amplitude propagating contractions, increasing bowel motility without significant activity at 5-HT2A, 5-HT2B, 5-HT3, motilin, or CCK-A receptors.
Plecanatide is a GC-C agonist and structural analog of human uroguanylin that acts locally on the luminal surface of the intestinal epithelium; activation of GC-C increases intracellular and extracellular cGMP, stimulating chloride and bicarbonate secretion via the CFTR ion channel to increase intestinal fluid and accelerate transit.
