Dosing
Diabetes Mellitus, Type 1 Approximately one-third of total daily insulin requirements SC once daily at the same time each day; use short-acting, premeal insulin to satisfy the remainder.
Diabetes Mellitus, Type 2 0.2 units/kg or up to 10 units SC once daily for insulin-naive patients; adjust based on metabolic needs and glucose monitoring results.
Diabetes Mellitus Inject subcutaneously in the abdominal region, buttocks, thigh, or upper arm twice daily within 15 minutes before meal initiation (or after meal initiation for type 2 patients); individualize dose based on metabolic needs, blood glucose monitoring results, and glycemic control goal.

Pharmacology
Insulin glargine is a long-acting human insulin analog that regulates glucose metabolism by stimulating peripheral glucose uptake in skeletal muscle and fat and inhibiting hepatic glucose production, providing a relatively constant concentration/time profile over 24 hours with no pronounced peak after subcutaneous injection.
Biphasic insulin analog consisting of 70% insulin aspart protamine (prolonged absorption) and 30% soluble insulin aspart (rapid absorption); lowers blood glucose by stimulating peripheral glucose uptake in skeletal muscle and fat and by inhibiting hepatic glucose production.
