Indications

Dosing
Erectile dysfunction (vasculogenic, psychogenic, or mixed etiology) Initiate at 2.5 mcg intracavernously; titrate in 2.5-10 mcg increments to effective dose (range 1-40 mcg); inject over 5 to 10 seconds; no more than 3 times weekly with at least 24 hours between doses.
Erectile dysfunction (neurogenic etiology) Initiate at 1.25 mcg intracavernously; titrate in 1.25-5 mcg increments to effective dose (range 1-40 mcg); inject over 5 to 10 seconds; no more than 3 times weekly with at least 24 hours between doses.
Erectile Dysfunction 50 mg orally as needed approximately 1 hour before sexual activity (30 minutes to 4 hours prior); may increase to 100 mg or decrease to 25 mg based on effectiveness and toleration; maximum once daily. Consider starting dose of 25 mg in patients >65 years, hepatic impairment, severe renal impairment, or concomitant use of ritonavir or strong CYP3A4 inhibitors.
Contraindications
- Conditions predisposing to priapism, such as sickle cell anemia or sickle cell trait, multiple myeloma, or leukemia
- Fibrotic conditions of the penis, such as cavernosal fibrosis or Peyronie's disease
- Penile implants
- Concomitant use of nitric oxide donors, including organic nitrates or organic nitrites in any form
- Known hypersensitivity to sildenafil or any component of the tablet
- Concomitant use of guanylate cyclase (GC) stimulators, such as riociguat
Adverse Reactions

Pharmacology
Prostaglandin E1 (PGE1) smooth muscle relaxant; induces penile erection by relaxing trabecular smooth muscle and dilating cavernous arteries, expanding lacunar spaces and entrapping blood via the corporal veno-occlusive mechanism.
Sildenafil is a selective inhibitor of phosphodiesterase type 5 (PDE5) that enhances the effect of nitric oxide by preventing degradation of cyclic guanosine monophosphate (cGMP) in the corpus cavernosum, resulting in smooth muscle relaxation and increased blood inflow during sexual stimulation; it has no effect in the absence of sexual stimulation.
