Dosing
Exocrine pancreatic insufficiency (adults and pediatric >=4 years) Initial 500 lipase units/kg/meal orally; titrate up to 2,500 lipase units/kg/meal, 10,000 lipase units/kg/day, or 4,000 lipase units/g fat ingested/day.
Exocrine pancreatic insufficiency (pediatric >12 months to <4 years) Initial 1,000 lipase units/kg/meal orally; titrate up to 2,500 lipase units/kg/meal, 10,000 lipase units/kg/day, or 4,000 lipase units/g fat ingested/day.
Exocrine pancreatic insufficiency (birth to 12 months) 3,000 lipase units per 120 mL of formula or per breastfeeding, administered orally immediately prior to each feeding.
