Dosing
Hypertensive disease Adults: 5 mg once daily orally, titrated up to 40 mg daily; pediatric patients >1 month: 0.08 mg/kg (up to 5 mg) once daily; renal impairment (GFR <=30 mL/min) or dialysis: initial dose 2.5 mg daily.
Heart failure 2.5 mg twice daily orally, titrated up to 20 mg twice daily as tolerated; usually given in combination with diuretics and digitalis.
Ventricular Dysfunction, Left 2.5 mg twice daily orally, titrated up to 10 mg twice daily as tolerated.
Chronic Kidney Diseases, Heart failure 10 mg orally once daily.
Diabetes Mellitus, Non-Insulin-Dependent Starting dose 5 mg orally once daily; may be increased to 10 mg orally once daily for additional glycemic control. Not recommended when eGFR is less than 45 mL/min/1.73 m2 for glycemic control.
Contraindications
  • History of angioedema or hypersensitivity related to previous ACE inhibitor treatment
  • Hereditary or idiopathic angioedema
  • Co-administration of aliskiren in patients with diabetes
  • Combination with a neprilysin inhibitor (e.g., sacubitril), or within 36 hours of switching to or from sacubitril/valsartan
  • Previous serious hypersensitivity reaction to dapagliflozin or any excipient in FARXIGA
Adverse Reactions
Most common (>1%) Fatigue, orthostatic effects, asthenia, cough, rash; in heart failure patients: hypotension, dizziness.
Serious Angioedema, hypotension, hepatic failure, renal impairment, hyperkalemia, cardiac arrest, myocardial infarction, cerebrovascular accident, neutropenia, thrombocytopenia, bone marrow depression.
Postmarketing Pancreatitis, toxic epidermal necrolysis, Stevens-Johnson syndrome, pemphigus, eosinophilic pneumonitis, pulmonary infiltrates, gynecomastia.
Most common (>=2%) Female genital mycotic infections, nasopharyngitis, urinary tract infections, back pain, increased urination, male genital mycotic infections, nausea, influenza, dyslipidemia, constipation, discomfort with urination, pain in extremity
Serious Diabetic ketoacidosis, volume depletion, urosepsis and pyelonephritis, hypoglycemia with concomitant insulin or insulin secretagogues, necrotizing fasciitis of the perineum (Fournier's Gangrene), hypersensitivity reactions including anaphylaxis and angioedema
Postmarketing Necrotizing fasciitis of the perineum (Fournier's Gangrene), urosepsis and pyelonephritis, ketoacidosis, acute kidney injury, rash
Pharmacology
ACE inhibitor; enalapril is hydrolyzed to enalaprilat, which inhibits angiotensin-converting enzyme, suppressing the renin-angiotensin-aldosterone system and thereby reducing plasma angiotensin II, vasopressor activity, and aldosterone secretion.
SGLT2 inhibitor; dapagliflozin inhibits sodium-glucose cotransporter 2 in the proximal renal tubules, reducing glucose reabsorption and promoting urinary glucose excretion, while also reducing sodium reabsorption to lower cardiac pre- and afterload, suppress sympathetic activity, and decrease intraglomerular pressure via tubuloglomerular feedback.
