Dosing
Hypertensive disease Starting dose 40/12.5 mg or 20/12.5 mg once daily depending on prior monotherapy; titrate up to 40/25 mg if needed.
Hypertensive disease (adults) 5 mg orally once daily (max 10 mg); start 2.5 mg in small, fragile, elderly patients, or those with hepatic insufficiency.
Hypertensive disease (pediatric, ages 6-17) 2.5-5 mg orally once daily; doses >5 mg have not been studied in pediatric patients.
Stable angina, Angina Pectoris, Variant 5-10 mg orally once daily; lower dose suggested in elderly and patients with hepatic insufficiency; most patients require 10 mg.
Coronary Artery Disease 5-10 mg orally once daily; majority of patients required 10 mg in clinical studies.
Contraindications
- Hypersensitivity to any component of BENICAR HCT
- Anuria
- Coadministration with aliskiren in patients with diabetes
- Known sensitivity to amlodipine
Adverse Reactions
Most common (>2%) Dizziness, upper respiratory infection, hyperuricemia, nausea
Serious Hypotension, impaired renal function, hypersensitivity reactions, electrolyte and metabolic imbalances, acute myopia and secondary angle-closure glaucoma, systemic lupus erythematosus, sprue-like enteropathy
Postmarketing Asthenia, vomiting, hyperkalemia, rhabdomyolysis, alopecia, pruritus
Most common (>1%) edema, dizziness, flushing, palpitation, fatigue, nausea, abdominal pain, somnolence
Postmarketing gynecomastia, jaundice, hepatic enzyme elevations, extrapyramidal disorder
Pharmacology
Olmesartan medoxomil selectively blocks angiotensin II binding to the AT1 receptor in vascular smooth muscle, inhibiting vasoconstriction independent of angiotensin II synthesis pathways; hydrochlorothiazide is a thiazide diuretic that directly increases renal excretion of sodium and chloride, reducing plasma volume to lower blood pressure.
Amlodipine is a dihydropyridine calcium channel blocker that inhibits transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle, causing peripheral arterial vasodilation, reduction in peripheral vascular resistance, and reduction in blood pressure.
