Pharmacology
Olmesartan medoxomil selectively blocks angiotensin II binding to the AT1 receptor in vascular smooth muscle, inhibiting vasoconstriction independent of angiotensin II synthesis pathways; hydrochlorothiazide is a thiazide diuretic that directly increases renal excretion of sodium and chloride, reducing plasma volume to lower blood pressure.
Amlodipine is a dihydropyridine calcium channel blocker that inhibits transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle, acting as a peripheral arterial vasodilator to reduce peripheral vascular resistance and blood pressure, and relieving angina by reducing myocardial oxygen demand and blocking coronary vasospasm.
