Indications

Dosing
Hypertensive disease Initiate IV infusion at 1-2 mg/hour; double dose at 90-second intervals initially, then increase by less than doubling every 5-10 minutes as goal blood pressure is approached; maintenance 4-6 mg/hour; maximum 16 mg/hour (up to 32 mg/hour for severe hypertension); lipid load limit 1000 mL or an average of 21 mg/hour per 24 hours.
Hypertensive disease Once daily; dosage may be increased in 2-week intervals as needed; maximum recommended dose is 40/10/25 mg; dose selection should be individualized based on previous therapy.
Contraindications
  • Allergy to soybeans, soy products, eggs, or egg products
  • Defective lipid metabolism such as pathologic hyperlipemia, lipoid nephrosis, or acute pancreatitis accompanied by hyperlipidemia
  • Severe aortic stenosis
  • Anuria
  • Hypersensitivity to any component of Tribenzor or to other sulfonamide-derived drugs
  • Co-administration of aliskiren in patients with diabetes
Adverse Reactions
Most common Headache, nausea, vomiting, acute renal failure, atrial fibrillation
Serious Myocardial infarction, cardiac arrest, syncope, dyspnea
Postmarketing Increased blood triglycerides, ileus, hypersensitivity, hypotension, nausea, decreased oxygen saturation, reflex tachycardia
Most common (>=2%) Dizziness, peripheral edema, headache, fatigue, nasopharyngitis, muscle spasms, nausea, upper respiratory tract infection, diarrhea, urinary tract infection, joint swelling
Postmarketing Angioedema, anaphylactic reactions, rhabdomyolysis, acute renal failure, sprue-like enteropathy, hyperkalemia, alopecia, pruritus, urticaria, gynecomastia, jaundice, hepatic enzyme elevations, extrapyramidal disorder, non-melanoma skin cancer (hydrochlorothiazide)
Pharmacology
Clevidipine is a dihydropyridine L-type calcium channel blocker that lowers blood pressure by inhibiting calcium influx in arterial smooth muscle, reducing systemic vascular resistance without affecting venous preload or cardiac filling pressure.
Triple-combination antihypertensive; olmesartan medoxomil selectively blocks angiotensin II AT1 receptors in vascular smooth muscle, amlodipine inhibits transmembrane calcium ion influx into vascular and cardiac smooth muscle, and hydrochlorothiazide promotes renal excretion of sodium and chloride to reduce intravascular volume.
