Dosing
Iron deficiency anemia 30 mg orally twice daily on an empty stomach, 1 hour before or 2 hours after meals; swallow capsules whole; treat for at least 12 weeks or until ferritin levels normalize.
Hyperphosphatemia (disorder) Starting dose 2 tablets orally 3 times per day with meals; titrate in increments or decrements of 1 to 2 tablets per day at 1-week or longer intervals; maximum 12 tablets daily.
Iron deficiency anemia Starting dose 1 tablet orally 3 times per day with meals; titrate as needed to achieve and maintain hemoglobin at target levels; maximum 12 tablets daily.
Contraindications
  • Hypersensitivity to ferric maltol or any excipient (reactions may include shock, clinically significant hypotension, loss of consciousness, or collapse)
  • Hemochromatosis and other iron overload syndromes
  • Receiving repeated blood transfusions
  • Iron overload syndromes (e.g., hemochromatosis)

Pharmacology
ACCRUFER (ferric maltol) is an oral ferric iron complex that delivers iron for uptake across the intestinal wall, enabling transfer to transferrin and ferritin to replenish iron stores.
Ferric citrate acts via two mechanisms: ferric iron binds dietary phosphate in the GI tract and precipitates as insoluble ferric phosphate to lower serum phosphorus; and ferric iron is reduced to the ferrous form in the GI tract and absorbed into the blood, where it circulates bound to transferrin and can be incorporated into hemoglobin.
