Dosing
Clinically isolated syndrome, Multiple Sclerosis, Relapsing-Remitting, Multiple Sclerosis, Primary Progressive Initial dose: 300 mg IV infusion, followed two weeks later by a second 300 mg IV infusion; subsequent doses: 600 mg IV infusion every 6 months.
Clinically isolated syndrome, Multiple Sclerosis, Relapsing-Remitting, Multiple Sclerosis, Secondary Progressive Starting dose 120 mg twice a day orally for 7 days; maintenance dose 240 mg twice a day orally.
Contraindications
- Active HBV infection
- History of life-threatening infusion reaction to OCREVUS
- Known hypersensitivity to dimethyl fumarate or any excipient of TECFIDERA
Adverse Reactions

Pharmacology
CD20 antagonist; ocrelizumab is a recombinant humanized IgG1 monoclonal antibody that binds to CD20 on pre-B and mature B lymphocytes, resulting in antibody-dependent cellular cytolysis and complement-mediated lysis.
Dimethyl fumarate (DMF) and its active metabolite monomethyl fumarate (MMF) activate the Nuclear factor (erythroid-derived 2)-like 2 (Nrf2) pathway involved in the cellular response to oxidative stress; the mechanism by which DMF exerts its therapeutic effect in multiple sclerosis is unknown.
