Dosing
Overactive Bladder, Urinary Incontinence 4 mg orally once daily; reduce to 2 mg once daily for mild to moderate hepatic impairment (Child-Pugh A or B), severe renal impairment (CCr 10-30 mL/min), or concomitant potent CYP3A4 inhibitors; not recommended in severe hepatic impairment (Child-Pugh C) or CCr <10 mL/min.
Overactive Bladder 25 mg orally once daily (starting dose), alone or in combination with solifenacin succinate 5 mg once daily; may increase to 50 mg once daily after 4-8 weeks.
Neurogenic detrusor overactivity Pediatric patients >=3 years: weight-based once-daily dosing; patients <35 kg use MYRBETRIQ Granules (24-64 mg/day depending on weight); patients >=35 kg use MYRBETRIQ 25 mg once daily (or MYRBETRIQ Granules 6 mL/48 mg once daily), increasing to maximum 50 mg (or 10 mL/80 mg) after 4-8 weeks.
Contraindications
- Urinary retention
- Gastric retention
- Uncontrolled narrow-angle glaucoma
- Known hypersensitivity to tolterodine, its ingredients, or fesoterodine fumarate extended-release tablets
- Known hypersensitivity to mirabegron or any inactive ingredient of the tablet or oral suspension
Adverse Reactions
Most common (>=1%) dry mouth, headache, constipation, abdominal pain, dyspepsia, somnolence, xerophthalmia, fatigue, dizziness, sinusitis, vision abnormal, anxiety, dysuria

Pharmacology
Tolterodine is a competitive antagonist of acetylcholine at postganglionic muscarinic receptors; it and its active metabolite 5-hydroxymethyl tolterodine (5-HMT) reduce urinary bladder contraction and overactive bladder symptoms by blocking cholinergic muscarinic signaling.
Mirabegron is a beta-3 adrenergic receptor agonist that relaxes detrusor smooth muscle during the bladder storage phase by activating beta-3 AR, thereby increasing bladder capacity.
