Dosing
Parkinson Disease Administer via continuous 16-hour intrajejunal infusion through a PEG-J tube using the CADD-Legacy 1400 pump; dose individualized as a morning dose, continuous dose, and extra doses; maximum 2000 mg levodopa (one cassette) per day.
Parkinson Disease 20 mg orally once daily; may increase to 40 mg once daily based on individual need and tolerability; may be taken with or without food; max 20 mg once daily with strong CYP3A4 inhibitors or moderate hepatic impairment; 40 mg once daily in tobacco smokers (>=20 cigarettes/day); avoid with strong CYP3A4 inducers or in severe hepatic impairment.
Contraindications
- Concurrent or recent (within 2 weeks) use of nonselective MAO inhibitors (e.g., phenelzine, tranylcypromine)

Pharmacology
Carbidopa inhibits peripheral decarboxylation of levodopa, increasing levodopa availability in the brain; levodopa crosses the blood-brain barrier and is converted to dopamine, which is thought to treat the motor symptoms of Parkinson's disease.
Adenosine A2A receptor antagonist; istradefylline is a xanthine derivative demonstrated in vitro and in animal studies to block adenosine A2A receptors, though the precise mechanism of therapeutic action in Parkinson's disease is unknown.
