Dosing
Parkinson Disease Administer via continuous 16-hour intrajejunal infusion through a PEG-J tube using the CADD-Legacy 1400 pump; dose individualized as a morning dose, continuous dose, and extra doses; maximum 2000 mg levodopa (one cassette) per day.
Parkinson Disease 50 mg orally once daily at bedtime; avoid food 1 hour before and at least 1 hour after dose; reduce to 25 mg in moderate hepatic impairment (Child-Pugh B); avoid use in severe hepatic impairment (Child-Pugh C).
Contraindications
- Concurrent or recent (within 2 weeks) use of nonselective MAO inhibitors (e.g., phenelzine, tranylcypromine)
- Concomitant use of non-selective monoamine oxidase (MAO) inhibitors
- Pheochromocytoma, paraganglioma, or other catecholamine secreting neoplasms
Adverse Reactions

Pharmacology
Carbidopa inhibits peripheral decarboxylation of levodopa, increasing levodopa availability in the brain; levodopa crosses the blood-brain barrier and is converted to dopamine, which is thought to treat the motor symptoms of Parkinson's disease.
Opicapone is a peripheral, selective and reversible inhibitor of catechol-O-methyltransferase (COMT) that reduces levodopa breakdown to 3-OMD, thereby increasing levodopa exposure in Parkinson's disease patients treated with levodopa/carbidopa.
