Pediatric <45 kg (>8 years, less severe disease): 5.3 mg/kg divided in 2 doses on day 1, then 2.6 mg/kg daily; severe or life-threatening infections: 2.6 mg/kg every 12 hours. Pediatric >=45 kg: adult dose.

Malaria Adults: 120 mg once daily. Pediatric >=8 years: 2.4 mg/kg once daily; >=45 kg: adult dose. Begin 1-2 days before travel, continue during travel, and for 4 weeks after leaving the malarious area.
