•Patients who qualify may be enrolled for up to 1 year
•Enrollment verification process begins when physician faxes application form and valid prescription to: 18442693038; once enrolled, a Takeda Oncology Here2Assist case manager will notify patient and healthcare provider
•1month supply of the medication will be delivered to the patient at no cost; each month a Takeda Oncology Here2Assist case manager will confirm that the patient is still being treated and is eligible to receive another month’s supply of the medication
