•Eligible patients may submit a rebate request form if they already paid in full for treatment while eligible for the program but not enrolled

•Offer valid for the outofpocket cost for the dose of AVEED only, and does not include other products or other outofpocket costs (for example, office visit charges, office visit copays, or injection/administration costs)
