•Program covers only the cost of the drug and does not cover copays related to administration, office visits, or any network penalties levied by patient’s insurance

•Maximum saving limit applies; patient outofpocket expense may vary on a monthly, quarterly, and/or annual basis depending on each individual patient’s plan of insurance and other prescription drug costs

•Reenrollment not necessary; patients remain enrolled unless there is a change in insurance or treatment status

•Depending on patient income or, in some cases, if medication is obtained from an outofnetwork provider, there may be an annual cap that limits the amount of assistance per year; federal and state laws and other factors may prevent or otherwise restrict eligibility
