The Calquence Patient Savings Program is designed to help eligible patients reduce the costs of their Calquence medication. With the Calquence Patient Savings Program, commercially insured patients may qualify to pay $0 for a 30-day supply of Calquence, providing substantial savings and access to necessary treatment. This program, however, is exclusively available to U.S. residents with commercial insurance and is not applicable to those with federally funded insurance plans.

•Program covers the cost of the drug only, and does not cover costs for office visits, or any other associated costs

•Must have a valid prescription

•Patients can complete an enrollment form with their HCP and fax to 18443292360

•HCP may also enroll patients online at https://www.calquencesavings.com/

•Offer is invalid for claims or transactions more than 365 days from the date of service
