•Vertex Connects is a support program designed to provide patients with dedicated support programs to help access their Casgevy therapy

•Once Casgevy is prescribed, patient will be provided with enrollment form

•A completed Vertex Connects enrollment form is needed to participate in the program and determine eligibility for certain program offerings; however enrollment in Vertex Connects is not required to receive a Vertex gene therapy
