Through this program, eligible commercially insured patients can pay as little as $0 for their 30- or 90-day prescriptions of Creon. The CFCareForward Co-Pay Card also provides an annual co-pay assistance benefit of up to $8,000, with no need for re-enrollment each year. •Restrictions, including monthly maximums, may apply

•Subject to all other terms and conditions, the maximum annual benefit that may be available solely for the patient’s benefit under the copay assistance program is $8,000.00 per calendar year

•Benefit may vary on a monthly, quarterly, and/or annual basis, depending on each individual patient’s plan of insurance and other prescription drug costs

•Copay card resets each January 1st; no reenrollment is required

•Cystic fibrosis patients can also receive nutritional support and multivitamins at no charge
