•HCP first must submit the CYSTADANE treatment form. to initiate process (fax to: 18558132039)

•The specialty pharmacy Anovo will work start on benefit investigation and prior authorization support, if needed, to determine patient's eligibility for copay program

•If approved, patient will receive medication shipped directly to their home every month

•Program will call patient to confirm if refills are needed
