Program Details

•Commercially insured patients with an out of pocket costs associated with EMFLAZA may be offered a copay assistance program

•HCPs should fax a completed 'Prescription Start Form' to: 18443229980, including patient, insurance, and HIPAA authorization

•Once prescription start form has been received by EMFLAZA Cares, a patient support specialist will be in touch with the patient/caregiver to review current insurance coverage, provide information about the qualifications for and benefits from patient assistance programs, and make arrangements for EMFLAZA delivery

•Patients can call the helpline directly (18444782227) to learn more about the patient support services and speak to a nurse case manager
