Program Details
•Patient must be enrolled in Gamifant Cares Support Services to initiate insurance, benefits verification, coordinate product orders through Specialty Pharmacy or Specialty Distributor, and assess patient’s eligibility for financial assistance
•All completed forms should be faxed to 18668957204 or emailed to GamifantCares@rxallcare.com
•Must also include copies of patient's insurance and pharmacy benefit cards
•Once the start form has been received, a member of the Gamifant Patient Services team will contact HCP to facilitate the ordering process and help ensure patient receives their treatment as quickly as possible
