•Eligible commercially insured patients may pay as little as $0 per prescription
•Maximum benefit of up to $18,000 per year
•Depending on patient's insurance plan, they could have additional financial responsibility for any amounts over Eisai’s maximum liability
•Patient and HCP should complete enrolment form and fax to 8447452350 or mail to: Halaven $0 Savings Program, 2250 Perimeter Park Drive, Suite 300, Morrisville, NC 27560
•If eligible to participate in the HALAVEN $0 Savings Program, a welcome letter, with information card, will be mailed to the patient and faxed to the physician
•If the patient is not eligible for the HALAVEN $0 Savings Program, a denial notification will be sent to the patient and faxed to the physician
•Program is valid for one year; after one year, a new application must be submitted
•May not be combined with any other coupon, discount, prescription savings card, free trial or other offer
