Program Details

•LLS covers all products prescribed by the patient’s physician to treat their primary cancer diagnosis classified as a chemotherapy or prescription related to their covered diagnosis
•All products must be covered by the patient’s primary insurance provider
•Patients, caregivers, pharmacies, and healthcare providers can apply by phone or online
•Patients must have a valid email address to register & providers will need their NPI
•Once registered, patients will receive an email directing to a website to create a password; application form can then be entered
•To apply, patient will need to supply SSN or Alien ID, DOB, diagnosis, insurance details (group, policy, and ID numbers), Medicare/Medicaid info, physician, and pharmacy details
•Once the application is submitted, the approval/denial or pending status will be provided immediately
•If the application is approved, the patient can begin using the award right away with the virtual pharmacy copay card that will be attached to the award message
•LLS will fax a physician's form to the treating HCP; the completed form must be received within 30 days of the approval, or the award will be closed, and no further claims may be submitted
•Any required documentation can be uploaded to the portal, as well as claim submissions and payment requests
