The Patient Advocate Foundation Co-Pay Relief Diabetes Type 2 fund covers co-pay, co-insurance, and deductible costs for eligible patients with type 2 diabetes. Currently closed to new applications. Starting July 2026, PAF launches TotalAssist as a unified replacement.

Program Details
Covers copay, coinsurance, and deductible for therapeutic diabetes medications
Also covers qualifying therapeutic devices and certain diabetes monitoring tests and office visits
Household income must be at or below 400% of the Federal Poverty Guideline
Available to patients with Medicare, Medicaid, Military Benefits, or commercial insurance
Currently closed to new and renewal applications due to lack of sufficient funding
Starting July 1 2026 PAF plans to launch TotalAssist as a unified financial assistance program
