•HCP or patient's doctor must submit the enrollment form by faxing it to 18778587746

•SPARK reimbursement specialists will review patients' eligibility for financial assistance programs if they cannot afford care

•Upon approval, SPARK will send eligible patients' prescriptions to a specialty pharmacy

•The pharmacy will get in touch with patients directly and arrange to have Korlym shipped to their homes

•The pharmacy will also contact the HCP/doctor and patients when their prescriptions need to be refilled

•Uninsured patients through the Korlym Patient Assistance Program may be referred to independent nonprofit organizations
