•Eligible commercially insured patients may pay $0 copay per prescription fill

•Eligible commercially insured patients may also be able to receive reimbursement up to $900 for covered initial assessments/examinations and firstdose observations associated with beginning treatment

•Patient pays a $125 deductible before reimbursement may be provided

•Medical copay support offer is for covered initial assessments/examinations and firstdose observations (FDOs) provided without regard to whether the patient continues on with MAYZENT therapy
