Program Details

•Program covers costs of copayments or coinsurance at 100%, up to the program maximum, for eligible patients

•Once doctor has prescribed Myalept, Specialty Pharmacy will do a benefit verification with insurance company to identify outofpocket costs

•Pharmacy then contacts patient with copay amount and delivery schedule

•Patients not eligible to participate in the ByMySide Copay Program may be referred to thirdparty patient assistance programs
