Program Details

•Physician must be Myalept REMS certified, and must begin the process by submitting a prescription and enrollment form to the program on behalf of the patient (faxed to: 18773289682)

•Program then sends enrollment packet to patient

•Once packet is completed and returned to program, patient is referred to ByMySide, who administers the assistance program

•ByMySide will then reach out to patient to begin assistance

•Medication is shipped to patient or physician for qualifying patients

•Program provides a nurse training visit with first shipment
