Program Details

•Eligible patients may pay $0 copay

•The Copay Program Maximum for patients who are enrolled in most commercial insurance plans is $9,450 annually

•The Copay Program Maximum for patients determined to be enrolled in high deductible health plans is up to $15,000 annually

•For certain eligible patients, the NUCALA Copay Program covers up to $100 patient cost for each inoffice administration, subject to the program maximum

•Doctor's office visits, labs, and other ancillary services are not included in the CoPay Program

•Residents of Massachusetts, Minnesota, and Rhode Island are not eligible for reimbursement of administrative fees
