Program Details

•Eligible patients receive medication free of charge for up to one year

•Program expires 12 months from approval date, unless patient has Medicare, which requires reapplication every January

•Medication is shipped directly to physician's office or patient's home in 90day supply

•Every year, patients must reapply, and be approved, to continue in the program
