Program Details

•Patients with commercial insurance that covers Nuplazid with a copay or coinsurance can get Nuplazid for less than $10

•Eligible patients may enroll in the Acadia Connect Copay Assistance Program through the Specialty Pharmacy or Acadia Connect

•Must have a valid prescription

•For patients enrolled in Acadia Connect, Acadia Connect Care Coordinators will automatically enroll commercially insured patients who may have a copay or coinsurance amount for the copay assistance program

•Patients can enroll online, or download the PDF form and email to acadiaconnect@acadiapharm.com

•Once verified and enrolled in the Acadia Connect Copay Assistance Program, Acadia Pharmaceuticals Inc. will cover 100% of the eligible outofpocket drug costs after insurance benefits are paid

•Only prescription drug costs defined as the applicable copay and/or coinsurance required by the patient to pay for an onlabel prescription of NUPLAZID are eligible for reimbursement under this program

•Transportation and appointment fees are not eligible for reimbursement under this program
