Program Details

•Eligible patients whose commercial insurance plan does not cover OPZELURA will pay $35 per prescription and will only be able to redeem this offer at certain participating pharmacies

•Maximum saving limit applies; patient outofpocket expense may vary on a monthly, quarterly, and/or annual basis depending on each individual patient’s plan of insurance and other prescription drug costs

•Patient or HCP may submit request for copay card online and receive information after selecting patient's condition and answering a few basic eligibility questions
