    •Valid only for residents in the US and Puerto Rico

    Program Details
    •Eligible commercially insured patients may pay as little as $25 for a 1month, 2month, or 3month supply of Ozempic
    •Maximum savings of $100 for a 1month prescription, $200 for a 2month prescription, and $300 for a 3month prescription
    •In order to obtain the “PALA $25 per 3month prescription” offer, the patient must have a prescription, written and dispensed for a 3month supply, and the patient’s commercial insurance plan must provide coverage for a 3month fill
