Program Details
•PROvide program supports eligible patients with private commercial (nongovernment payers) insurance by covering any combination of cost—copays, coinsurance, or deductible costs—to a maximum of $10,000 over 3 PROVENGE treatments

•Explanation of Benefits must be submitted to program within 90 days of each treatment in order to receive reimbursement
