Program Details

•HCP must complete and submit a Start Form for the patient before they can speak with a Care Navigator and access REACH support offerings

•Patient is not required to enroll in REACH before HCP prescribes a Reata medicine

•Patient must provide information to complete the patient sections of the form

•The Start Form should be faxed to REACH at 18448061718

•**Note: If any part of the form that is received by the specialty pharmacy is incomplete or missing, Reata REACH will contact HCP or patient for clarification and completion

•REACH may contact the patient to send a digital consent form; patient signature is not required to submit and process a prescription
