Program Details

•Pay as little as $25 for a 1month supply of Taltz (28 days, up to 3 pens)

•Must have commercial drug insurance that does NOT cover Taltz

•Requires prior authorization (PA) before first prescription fill

•If PA is denied, appeal must be submitted before 5th month fill

•New PA, appeal, or medical exception required before 13th fill to remain eligible

•Maximum of 14 prescription fills per calendar year

•Card expires 12/31/2026 or 24 months after first use, whichever comes first

•Not valid for patients enrolled in Medicare, Medicaid, TRICARE, VA, or other government programs

•Must be 18 years or older and a US or Puerto Rico resident
