•Annual assistance is limited to a maximum of $20,000
•Patients must complete the Enrollment and Consent form with their doctor and fax to Vertex at 18889525933
•Patient will then speak to a Case Manager to determine assistance, which will be verified after a benefits investigation
•Vertex GPS will confirm outofpocket costs for SYMDEKO, KALYDECO, TRIKAFTA or ORKAMBI and inform patients about available copay assistance programs offered
•Vertex GPS will work with patient’s pharmacy and send refill reminders
•Patient is automatically reenrolled as long as the therapy continues
