The AZ&Me Prescription Savings Program provides eligible patients with access to various medications, including Fasenra, Fasenra Pen, Airsupra, Farxiga, Xigduo XR, Saphnelo, Imfinzi, Lynparza, Truqap, Calquence, Imjudo, Imfinzi & Imjudo, Tagrisso, Lokelma, and Wainua, at no cost. This program, AZ&Me, is designed to help U.S. residents who meet specific criteria, including income thresholds, to receive necessary drugs without struggling with financial barriers. AZ&Me makes it easy for healthcare professionals to apply on behalf of their patients, ensuring essential medications are delivered conveniently to patient's homes or doctors' offices.

Program Details

•Correct medication must be selected before patient/HCP is able to download a paper application

•Eligible patients may be able to receive medication free of charge

•HCP should fax completed forms to 18772390867

•Medicine will be shipped either to patient’s home or doctor’s office
