Program Details

•Eligible commercially insured patients may pay as little as $0 per prescription to cover the patient’s copay or coinsurance

•Maximum benefit of $25,000 during a 12month enrollment period

•If the patient utilizes the entire $25,000 benefit throughout the 12 month calendar year period, the patient or healthcare provider may be able to request an exception due to financial hardship. These will be reviewed on a case by case basis. Approval will be determined by the Manager of Reimbursement Services of DSI

•To continue getting copay assistance, patients must reapply for the program at the start of each year

•Patient and doctor can enroll by downloading, printing, and filling out the Patient Enrollment Form and sending it to Biologics
