Program Details
Eligible uninsured, underinsured, or Medicare patients who are unable to meet their outofpocket costs may be able to get Turalio
Medicare Beneficiaries: must not be eligible for or enrolled in Low Income Subsidy (LIS) for Medicare Part D, and must have spent at least 3% of their annual household income on prescription medicines in the current year
Patient and doctor should complete the Patient Enrollment Form and fax to DSI Access Central at 18334719988 (form does not need to be sent to Biologics for patients eligible for PAP)
