•Valid only for residents in the US and Puerto Rico

•Enrollment Required: Yes

Program Details

•Program offers free medication for eligible patients through the end of the calendar year

•Prior to an application being completed, patients should call 18668965674 to complete an eligibility prescreening

•Patient must fill out enrollment form, including input from physician, and fax to 18009104610

•If a patient is accepted into the program, they will receive up to 3 months of medication per shipment, and up to 18 months of therapy

•Acceptance into the program is valid for the current calendar year and with a valid prescription (subject to continued eligibility) and until the patient has reached a total of 18 months of therapy provided by Radius Assist

•Radius Assist will discontinue providing therapy once the patient’s cumulative lifetime use of TYMLOS has reached 24 months

•Patients and prescribers will be notified by phone or mail of the approval or denial of the application
