Program Details

•Eligible commercially insured patients may pay as little as $0 outofpocket for each qualifying prescription

•Maximum benefit of $16,000 per calendar year

•Eligible commercially insured patients may also be able to receive a onetime rebate of up to $2,500 for baseline assessments/prescreening tests for the initial blood tests, ECG screening, eye exam, and baseline skin examination where the full cost is not covered by patient's insurance

•To receive reimbursement for qualified outofpocket expenses, an Explanation of Benefits (EOB) form must be submitted, along with copies of receipts for any payments made

•Patients residing in Massachusetts, Michigan, Minnesota, or Rhode Island are not eligible for the AtHome Baseline Assessment/Prescreening Tests Program

•Maximum saving limit applies; patient outofpocket expense may vary on a monthly, quarterly, and/or annual basis depending on each individual patient’s plan of insurance and other prescription drug costs
