Program Details

•Eligible commercially insured patients may pay as little as $0 per 30day supply

•Maximum saving limit applies; patient outofpocket expense may vary on a monthly, quarterly, and/or annual basis depending on each individual patient’s plan of insurance and other prescription drug costs

•Benefit is valid for the patient’s outofpocket cost for the product only; not valid for any other outofpocket costs (for example, office visit charges or medication administration charges) even if such costs are associated with the administration of the product
