Program Details

•Healthcare provider must start the enrollment process by submitting the enrollment form by fax to 18559826341 or upload to eService portal

•Healthcare provider can also complete the online enrollment at: https://xtandihcp.iassist.com/

•Once approved, patient and physician will be notified, and a 30day supply of XTANDI will be shipped directly to patient’s home for each month they are eligible

•Enrollment for the Astellas Access Program may last up to one year; patients must reenroll at the end of the 12 months of assistance
