•Patients who are uninsured or underinsured may be able to receive Zynlonta for free if they meet the criteria for the Patient Assistance Program
•HCP should fax completed enrollment form to 8559153005, or by email the completed form to support@ADVANCINGPatientSupport.com
•HCP may also enroll patient online
•If enrolling a patient online, patient must fill out and submit a Patient Authorization form and a Patient Assistance Program Consent form
