Dosing
Edema, Chronic heart failure, Chronic Kidney Diseases, Nephrotic Syndrome 30 mg subcutaneously over the first hour followed by 12.5 mg per hour for the subsequent 4 hours (total 80 mg) via On-body Infusor applied to the abdomen; not for chronic use, replace with oral diuretics as soon as practical.
Chronic Kidney Diseases (Adults) Starting dose 0.8-1.6 g orally three times daily with meals based on serum phosphorus level; titrate by 0.8 g per meal at two-week intervals to achieve target serum phosphorus.
Chronic Kidney Diseases (Pediatric, >=6 years) Starting dose 0.8-1.6 g three times daily with meals based on body surface area (BSA) category; titrate based on BSA category at two-week intervals for 6 weeks then every 4 weeks as needed.
Contraindications
- Anuria
- History of hypersensitivity to furosemide, any component of the FUROSCIX formulation, or medical adhesives
- Bowel obstruction
- Known hypersensitivity to sevelamer carbonate, sevelamer hydrochloride, or any excipient
Adverse Reactions
Serious fluid, electrolyte, and metabolic abnormalities, ototoxicity, anaphylactic or anaphylactoid reactions, aplastic anemia, toxic epidermal necrolysis, Stevens-Johnson syndrome, interstitial nephritis, necrotizing angiitis
Other reported pancreatitis, jaundice, nausea, vomiting, tinnitus, hearing loss, vertigo, dizziness, headache, thrombocytopenia, agranulocytosis, leukopenia, rash, erythema multiforme, orthostatic hypotension, injection site erythema and pain, muscle spasm, restlessness
Most common (>5%) Vomiting, nausea, diarrhea, dyspepsia, abdominal pain, flatulence, constipation
Postmarketing Hypersensitivity, pruritus, rash, abdominal pain, bleeding gastrointestinal ulcers, colitis, ulceration, necrosis, fecal impaction, ileus, intestinal obstruction, intestinal perforation
Pharmacology
Furosemide is a loop diuretic that primarily inhibits sodium and chloride reabsorption in the proximal and distal tubules and the loop of Henle, producing high-degree diuresis independent of carbonic anhydrase inhibition or aldosterone.
Sevelamer carbonate is a non-absorbed phosphate-binding cross-linked polymer, free of metal and calcium, that binds phosphate in the gastrointestinal tract via ionic and hydrogen bonding, thereby decreasing phosphate absorption and lowering serum phosphorus concentrations.
